You've all seen these on your travels around the Net. Most just ask for your name, email address and a message.
An enquiry form is normally used as an alternative to email. Email is good but often the messages sent to you are vague and often missing the vital information required for you to progress the enquiry, so you send them a reply and ask for more information (or second guess what they want)... and the cycle starts all over again.
